IN THE CIRCUIT COURT

OF THE FIFTH JUDICIAL CIRCUIT

STATE OF FLORIDA,

Plaintiff,

MOTION TO DEFINE AND / OR CLARIFY CONDITIONS OF SUPERVISION

COMES NOW the Defendant, Your Name, pro-se pursuant to Florida Rules of

Criminal Procedure 3.700 (a), and respectfully moves this Honorable Court to define

and/or clarify the condition imposed in the above-styled case. In support of, the

Defendant states the following:

STATEMENT OF THE CASE

Date of Arrest:                                    01-01-2001

Date of Sentence:                   02-02-2001

Sentencing Judge:                   Honorable Judge (Who)

Term of Sentence:                   What was your sentence itself

Plea:                                        Guilty

Attorney at Sentencing:          Your Attorney’s Name ( If pro-se, put pro se)

Motion Hearing:                      Date Of Hearing That You Are Requesting Clarification Of

PORTION OF CONDITION TO BE DEFINED AND / OR CLARIFIED is, from

the Motion Hearing on Month , Day , before the Honorable Judge ______ _______. The

Defendant was heard on his motion for _____________________ (what was your motion

for ?) , which was therefore granted by the Court. The Defendant  ( I.E.) also had filed a

motion for relief in employment, being that of a family auto repair business where a

child, under the age of 18 resides. This petition was granted; being that the Defendant

would be supervised while the 18 month old child is present. The clarification the

Defendant respectfully requests by this Honorable Court, is, there will be minors under

the age of 18 present at the family owned auto repair business, to have their vehicles

serviced periodically. Will the Defendant still be permitted to be employed at this

business with these situations? There may be times the Defendant will be working at the

auto shop periodically, with/without the supervision of the owners, ( who? ) without the

18 month old daughter present, though may in fact have unexpected customers, at the age

of 16 and over present, having their vehicles serviced. There may be minors that may

come with their parents to have their vehicles serviced as well.

WHEREFORE, the Defendant respectfully requests this Honorable Court enter an

order modifying / clarifying the aforementioned condition of  supervision to indicate as

requested above, and to direct the Clerk of Court to forward a certified copy of the

defined condition to the Florida Department of Corrections, Probation and Parole, Officer

(Officer Name);                                  Inverness, Fl 34450, and to the Defendant at the

address below.

I Declare under the penalty of perjury that the foregoing facts and matters

are true and correct.

Executed this     day of February, 2007.

Respectfully Submitted,
